At the same time, Howard said a Jeep Liberty, driven by Cynthia Hartson, 44, of Audubon, was traveling east on Route 422 in the right lane.
According to Howard, the Grand Prix proceeded to pass another vehicle by driving on the berm and Holmes lost control of his vehicle when it re-entered the roadway.
The Grand Prix struck the guide rail in the median, then entered the eastbound lane of Route 422. The vehicle then entered the path of the Jeep Liberty, and struck it in the front with its right side.
Howard said both vehicles left the roadway, went down the steep embankment and came to rest close to the Valley Forge Baptist Church.
Fire and emergency personnel arrived on the scene and began treating all of the vehicle's occupants while helicopters were landing on Route 422 and in the church's parking lot.
Holmes, along with passengers Quentin Branch, 37, of the 1st block of Cresmont Street, and Earl Snyder, 19, of the 200 block of Hanover Street, both of Pottstown, were removed from the Grand Prix.
Hartson and with her daughter, Mollie Hartson, were removed from the Jeep Liberty.
All four adults were airlifted via Life Flight to Hahnemann University Hospital while the juvenile was airlifted via PennStar to the Children's Hospital of Philadelphia (CHOP), all with unknown injuries.
Both sides of Route 422 was closed for 60 minutes while State police and fire officials began their investigation.
Assisting at the scene was Upper Providence Police Department, Mont Clare Fire Company, Humane Fire Company, Friendship Fire Company, Trappe Ambulance.
